Services

We offer our services in a range of contexts depending on the required support and the person we are working with. These include:

  • One-on-one support to access mainstream community activities, events, and facilities.
  • One-on-one and two-on-one in-home support.
  • Supported Independent Living (SIL) services to enable people with disability to live in their home with the appropriate wrap-around supports to enable their independence.
  • A combination of all of the above.

We do not offer a one size fits all model because it doesn’t work!  Instead, we work with individuals to develop a unique support plan that focusses on preferred life experiences and outcomes tailored specifically for the person.

Empowered Futures works within a person- and family-centred model and commits to supporting people with a disability to:

  • Exercise their rights as any other member of society to participate freely in and contribute to their communities of choice.
  • Participate at all levels of service design and delivery and be actively involved in the decisions that affect their lives to whatever extent is desired and/or meaningful to them.
  • Have access to services and supports in line with their needs, skills, strengths, gifts and interests.
  • Experience the full range of roles available to all members of the community.
  • Experience the full range and benefit of being in a relationship with others including social and interest groups, friends, partners, family members and other members of society.
  • Be included and valued as members of their community; and the organisation actively and intentionally promotes a positive image of people with a disability in all organisational activities.
